Community Partnerships

Community-Driven Solutions to Housing

A cute family standing outside their home. The young boy is up in his mother's arms, happily gesturing outwardly.

Addressing housing challenges requires collaboration, shared investment, and a commitment to community-driven solutions. While Trinity Health Ann Arbor plays an important role in improving community health, we recognize that lasting housing solutions can only be achieved through strong partnerships.

By working alongside nonprofit organizations, housing developers, local governments, and community leaders throughout Washtenaw County, we are helping expand access to affordable housing, support housing stability, and improve health outcomes for individuals and families.

The organizations highlighted below represent key partners in Trinity Health Ann Arbor's housing strategy. Through these collaborative efforts, we are advancing innovative and sustainable solutions that strengthen neighborhoods, increase housing opportunities, and help create a healthier, more equitable community for all.

Shelter Association of Washtenaw County (SAWC) Recuperative Care Program

Shelter Association logo

Trinity Health Ann Arbor provides financial support for the Shelter Association of Washtenaw County's (SAWC) Recuperative Care Program (RCP), an initiative designed to improve health outcomes for individuals experiencing homelessness or housing instability while reducing preventable and costly hospital utilization. The program serves individuals who are medically vulnerable and often face complex physical and behavioral health challenges.

Recuperative care, also known as medical respite care, offers short-term residential support for people experiencing homelessness who are well enough to be discharged from the hospital but too ill or frail to safely recover on the streets or in a traditional shelter setting. By providing a safe place to heal, access to supportive services, and connections to ongoing health care and housing resources, the Recuperative Care Program helps participants achieve greater stability, supports recovery, and reduces avoidable emergency department visits and hospital readmissions.

Trinity Health Ann Arbor provides 33,572 square feet of leased space for $1 per year to House N2 Home, a nonprofit organization that transforms empty housing into welcoming homes for individuals and families rebuilding after homelessness. Working in partnership with case workers from more than 50 agencies throughout Washtenaw County, House N2 Home receives referrals for people moving into permanent housing and works directly with each client to understand their individual needs and preferences.

With the support of hundreds of volunteers, the organization provides and thoughtfully sets up furniture, household essentials, linens, kitchen items, décor and other necessities – helping ensure that individuals and families are not simply housed, but have a comfortable, functional place to call home as they begin their next chapter.

Washtenaw Housing Alliance (WHA)

Trinity Health Ann Arbor is proud to partner with the Washtenaw Housing Alliance (WHA), having helped establish the organization and continue to serve as its employer of record since its inception in 2000. In addition to providing ongoing annual financial support, Trinity Health Ann Arbor provides in-kind support through partial funding of employee benefit costs for WHA staff.

Through the Sister Yvonne Gellise Fund for Permanent Supportive Housing Services (Yvonne Fund), Trinity Health Ann Arbor also contributed $1 million to establish this community endowment at the Ann Arbor Area Community Foundation, creating a sustainable source of revenue for permanent supportive housing services in Washtenaw County. The WHA works to grow the Fund for the community’s benefit and the aim of preventing and ending homelessness.

The Washtenaw Housing Alliance is a coalition of more than 25 organizations working collaboratively to prevent and end homelessness across the county. By coordinating resources, aligning strategies and strengthening partnerships among service providers, WHA helps create a more effective and responsive housing system.

The Alliance also plays a key role in advancing the development of permanent affordable housing solutions, ensuring that more individuals and families have access to safe, stable and affordable places to call home.
